PUT YOUR PERFORMANCE ON DISPLAY!

Get registered for our upcoming pole Showcase! To become a performer, read the rules and regulations, read the category information, and pay the performer fee. There are step-by-step instructions to follow below. Once you have filled out the Showcase forms and uploaded your headshot and song of choice, you will be registered! Contact us below with any questions or concerns you have and we will be in touch. We can’t wait to see what you’ve been working on and look forward to watching you perform!

How To Become A Performer

Read the Rules & Regulations.
Pay the performer fee.
Fill out the form below (please do so ONLY when you have all of the information needed). Do NOT fill out half of the form and send. You will NOT be able to go back and fill out the remaining information.
Once your submittal has been received, you will be added to the “Flirt Fitness Studio® Showcase Performers” Facebook group where you can ask questions, receive updates, and chat with fellow performers!

Important Information

Saturday May, 4th @ 4pm (doors open at 3pm) at Flirt Fitness GR
This form must be submitted by midnight Wednesday, April 24th. NO EXCEPTIONS.
Flirt Fitness Studio® will add you to the Facebook performer group within 48 hours after all information has been submitted and received.

In person ticket sales only and tickets will not be available at the door.

I expressly acknowledge that the use of drugs or alcohol, or being under the influence of drugs or alcohol, before or during my participation in any activities, classes, parties and related events at/with Flirt Fitness Studio^® is done at my own peril and is not condoned, encouraged, or excused by Flirt Fitness Studio^® is any way. I further understand that Flirt Fitness Studio^® and or the instructors and contractors reserve the right to deny me or any other person participation before or during an activity if they find that person to be mentally or physically impaired by drugs or alcohol. Drug and alcohol use on Flirt Fitness Studio^® or other contractors’ premises is absolutely prohibited for safety reasons.I further acknowledge that I am ultimately responsible for my own safety during my participation in activities, classes and related events provided by Flirt Fitness Studio^®, including but not limited to the use of their equipment or facilities.

Permissions

I agree that Flirt Fitness Studio^® can take photographs or make images of me. I also agree by giving permission below Flirt Fitness Studio^® may use the images as listed here: 1. Grant Flirt Fitness Studio^® permission to hang photographs with my image IN THE STUDIO for an indefinite amount of time. As such, photographs may remain on the wall after I am no longer a student and/or instructor. 2. Understand I am giving Flirt Fitness Studio^® my permission to use photographs with my image ONLY within the walls of the Flirt Fitness Studio^®. 3. Understand Flirt Fitness Studio^® will not compensate me for the use of my image in any way. 4. Understand Flirt Fitness Studio^® WILL NOT use names when displaying photographs of students and/or instructors. 5. Understand any photograph I provide to Flirt Fitness Studio^® or its management for use in the studio may not be returned to me.
